Say your client calls and needs $100,000.
Both spouses are 68, on Medicare Part B and D, filing jointly, with modified adjusted gross income (MAGI) already at $210,000. Today’s first income-related monthly adjustment amount (IRMAA) tier starts at $218,001.
Take the whole $100,000 from the IRA and MAGI lands at $310,000, two tiers up. The couple pays $5,770 in Part B and Part D surcharges for one year. The income tax on those IRA dollars was always coming. The surcharge was not.
Cap MAGI at $218,000 and the same $100,000 comes from high-basis holdings in the taxable account, harvested losses against the gains, and the Roth only for what is left. In this illustration, the surcharge never happens.
- Set the cap, then plan against it. Cap MAGI at an IRMAA tier, a tax bracket, or an ACA credit threshold, and the funding plan respects the cap you set.
